Do you have diabetes? Join the free Diabetes Self-Management Program for people 60 and over or those who are disabled.
This 2 1/2 hour program meets every Wednesday from October 25th to November 29th, 2023 from 9:30 a.m. to 11:30 a.m. at Lutheran Social Services of Northeast Florida, 4615 Philips Highway, Jacksonville Florida 32207.
Diabetes Self-Management Program (DSMP) is an evidenced-based program developed by Stanford University. Participants in this interactive program learn techniques to manage their blood sugar, prevent or reduce disease related complications such as foot and vision problems, discover better nutrition and exercise choices, set goals, problem solve, and communicate effectively with family and health care providers. The live webinar or phone sessions meet once a week for six weeks.
