Our onsite partners provide comprehensive services to address various needs, including healthcare and community connections.

 

Together, we strive to ensure individuals and families in Northeast Florida receive holistic care to help them live an  empowered life.

CAN Community Health is committed to providing the highest quality of care to those affected by HIV, Hepatitis C, and other sexually transmitted diseases through outreach, integrated clinical practice, advocacy, education, and research.

WeCareJax advances and coordinates community-wide compassionate specialty care for the uninsured.

JaxCareConnect links patients without insurance to a Primary Care medical provider for ongoing care—all at a price they can afford, or at no cost at all.
